Upon Acceptance of my application for entry and membership in Julio Juarez Brazilian Jiu Jitsu, I here by freely agree to the following terms and conditions. For the purpose of this agreement, members or all prospective members of Julio Juarez Brazilian Jiu Jitsu shall be referred to herein as " Member". For the purpose of this Agreement , the Julio Juarez Brazilian Jiu Jitsu Waiver of Liability Agreement Shall be referred to as "Agreement".
Member assumes the risk of injury by participating in activities and or training of Brazilian Jiu Jitsu. Member represents to the administrators of Julio Juarez Brazilian Jiu Jitsu that member is physically fit to perform such vigorous and strenuous activities without any threat to Member's health and safety. Member fully acknowledges the dangers of participating in Brazilian Jiu Jitsu activities and assumes all risks, whether obvious or latent. All exercise is done at the risk of Member and therefore Julio Juarez Brazilian Jiu JItsu is not liable for personal injury, accident, or death. Member also agrees to participate in accordance with the rules and judgments of the management and instructors, and understand that such decisions in all matters are final.

Julio Juarez Brazilian Jiu Jitsu strongly advises all Members to obtain a physical examination from his or her physician prior to participating in any Brazilian Jiu Jitsu activities.

Member has no physical disability, impairment, or ailment preventing Member from engaging in any exercise or activity that will be detrimental to the health, safety, comfort or condition of the Member or others.
Member acknowledges that Brazilian Jiu Jitsu activities may require that two (2) or more individuals, male or female, work together in close physical contact. Member acknowledges that such contact is solely for instructional purposes and that such physical contact is in no way any form of sexual harassment.
